Sprayberry Winter Guard Program

Each student joining the winter guard program will need to complete the registration process at the designated registration time. A parent will need to be present at registration. At registration students and parents will need to complete a packet of paperwork which will include parent contact information, medical information, and Cobb County Permission forms. All winter guard students are required to complete forms annually. All forms from previous years will be shredded at the end of each school year. **If your student is in the 2016-2017 Marching Band program we already have all the forms on file, thus you do not need to complete them again. **

Student/Parent Information Form
It is critical for the band directors to have up-to-date information for each student and parent.
Please complete the medical form in full and be sure to write "n/a" or "none" on items that are not applicable. This form needs to be notarized. A complimentary notary will be available at Registration. Parents must bring a photo ID to use this service.
Proof of Insurance or NO Insurance Form
Please bring a copy of both sides of your student's current medical insurance card. If your child is not covered by medical insurance, the "No Student Insurance" form must be completed.
Winter Guard Student Contract
A new Winter Guard Student Contract is required every year. Please read, review with your student and then both of you sign this form where indicated.
Field Trip Form
"Blanket" permission to participate in a series of school sponsored Field Trips form is required by Cobb County. This form will cover student participation in activities such as, but not limited to: band camps, camp activities, travel to away games, competitions, etc.
Permission to Photograph
This document grants permission to the Sprayberry High School Band of Gold and the Sprayberry Band Boosters Association (SBBA) to publish photos or videos of your student. (Required by Cobb County)
Permission to Swim Form
From time to time, the Sprayberry Band (or portion thereof) will have activities and/or go on trips where swimming will be made available to our students. Parent must grant permission to participate. (Required by Cobb County)
The Sprayberry Winter Guard receives a small amount of money from the band booster organization, but this amount is not nearly enough to cover the multiple expenses faced by fielding a winter color guard program. Student costs will be held to the lowest level possible to effectively run the winter program. Program fees are used to cover these items: Staff Instruction, Competition Registration, Equipment Purchase (Rifles, Sabers), Flags/Flag Design, Props and Paint, Uniforms, Hotel for SAPA Championships (Chattanooga), Food for SAPA Championships, Travel Expenses for equipment transport (Fuel, Parking, etc.). A minimum down payment is required at Registration from each student joining the winter guard program. In addition any accessory fees must be paid at Registration. Also at this time each student will need to commit to the designated fee schedule for each guard team and sign a Financial Commitment Agreement for the remaining program fee balance. Please have your first payment ready at the time of registration.

**Please note that all uniforms and equipment (marching, concert and guard) are property of the Sprayberry Band Program. Accessories are the property of the individual student. Uniform, equipment, and accessory expenses must be paid at Registration. **
Discounts (Applies only to Program Fees):
Sibling discount - 25% (for second and subsequent student in one household)
Paid in full discount - 10% (if all fees paid at time of Registration or via Direct Pay by Dec 15, 2016)

3 Month Plan - Monthly payments from January 2017 – March 2017. Remaining balance paid in full by April 1, 2017.

Direct Bill Pay (PREFERRED) – parent must arrange with their bank to make recurring payments to be mailed to: SBBA, PO Box 671051, Marietta, GA 30066.
Credit Card – at Registration and monthly SBBA Booster meetings.
Cash – at Registration, monthly SBBA Booster meetings, or deposited in the black lock box located in the band room (adjacent to the Band Director’s office).

Please do not let any financial issues stop you from registering your student for the band program. We will gladly work with you to address any financial questions/concerns you may have when you stop by the Accounting station at Registration. Fundraising activities are available to all band families to help with fees.
